Dans le contexte actuel de pluralisme idéologique généralisé, l'individu peut sans doute choisir d'être chrétien ou de ne pas l'être, mais que signifie dans un tel contexte l'étiquette « chrétienne » accolée à des institutions héritées d'un passé qui semble largement révolu ? La question se pose aussi pour les écoles, l'université, les partis politiques, mais elle présente sans doute une acuité plus grande lorsqu'il s'agit d'institutions sociales et de santé. Y a-t-il une santé chrétienne ? Un hôpital chrétien, ou une crèche, ou un home pour personnes âgées, sont-ils différents de leurs équivalents sans référence religieuse ? Celle-ci joue-t-elle encore un rôle ? La Confédération chrétienne des institutions sociales et de santé, qui rassemble en Belgique francophone les institutions d'obédience catholique, représente une part notable du secteur. Ses responsables ont voulu en avoir le cœur net. Au risque d'obtenir des résultats inattendus, ils ont confié la recherche à des équipes universitaires de chercheurs en sciences sociales, accompagnées par un petit groupe de théologiens. Utilisant des méthodes différentes (enquêtes, analyses qualitatives, « recherche-action »), les chercheurs ont interrogé les usagers de ces institutions, leur personnel, leurs gestionnaires. Le présent ouvrage publie les résultats de cette recherche, mais également les problèmes méthodologiques qu'elle a posés dès le départ. En effet, comment définir l'identité chrétienne ? Et encore, ne fallait-il pas s'interroger sur les motifs politiques qui peuvent guider une telle recherche ? N'est-elle pas motivée par la volonté de renforcer une image de marque, en termes de marketing ? Cette vigilance critique a accompagné tout le processus de la recherche, et se manifeste dans les résultats et les propositions qui les accompagnent. Le processus d’émergence, de structuration et d’institutionnalisation de la nouvelle médecine clinique, dite moderne, qui prend place en Europe à la fin du XVIIIe et au début du XIXe siècle représente une rupture sans précédent avec une tradition hippocratico-galénique vieille de près de vingt-trois siècles. La recherche, l’enseignement et la pratique ont désormais pour cadre central l’espace hospitalier, qui devient le champ principal de l’expérience clinique. Se démarquant résolument des thèses généralement reçues, la présente étude démontre que ce processus ne saurait être réduit à ce qui s’est passé à Paris, après la Révolution et la fin de l’année 1794, ni à ce qui a été opéré par ce qu’il est convenu d’appeler l’École Clinique de Paris au cours des deux ou trois premières décennies de son existence. C’est en fait par un jeu complexe d’interactions et d’échanges constants de modèles (de politique médicale d’un côté, de techniques et de concepts de l’autre) entre les diverses écoles médicales des différents pays européens que s’est opérée cette révolution scientifique capitale de la médecine, révolution qui touche autant les savoirs que les pratiques.

Thanks to the omnipresent computer, current statistics can include data files of many thousands of values, and can perform any exploratory analysis in less than seconds. This development, however fascinating, generally does not lead to simple results. We should not forget that clinical studies are, mostly, for confirming prior hypotheses based on sound arguments, and the simplest tests provide the best power and are adequate for such studies. In the past few years the authors of this 5th edition, as teachers and research supervisors in academic and top-clinical facilities, have been able to closely observe the latest developments in the field of clinical data analysis, and they have been able to assess their performance. In this 5th edition the 47 chapters of the previous edition have been maintained and upgraded according to the current state of the art, and 20 novel chapters have been added after strict selection of the most valuable and promising novel methods. The novel methods are explained using practical examples and step-by-step analyses readily accessible for non-mathematicians. All of the novel chapters have been internationally published by the authors in peer-reviewed journal, including the American Journal of Therapeutics, the European Journal of Clinical Investigation, The International journal of Clinical Pharmacology and therapeutics, and other journals, and permission is granted by all of them to use this material in the current book. We should add that the authors are well-qualified in their fields of knowledge. Professor Zwinderman is president-elect of the International Society of Biostatistics, and Professor Cleophas is past-president of the American College of Angiology. From their expertise they should be able to make adequate selections of modern methods for clinical data analysis for the benefit of physicians, students, and investigators. The authors, although from a different discipline, one clinician and one statistician, have been working and publishing together for over 10 years, and their research of statistical methodology can be characterized as a continued effort to demonstrate that statistics is not mathematics but rather a discipline at the interface of biology and mathematics. They firmly believe that any reader can benefit from this clinical approach to statistical data analysis.

Au cours des années 1960, les pays occidentaux s’engagèrent, avec des rythmes et des conceptions variés, dans un processus politique, médical et administratif visant à sortir les malades des hôpitaux psychiatriques. En opposition au modèle asilaire développé au xixe siècle, cette désinstitutionnalisation valorisait une prise en charge de la maladie mentale fondée sur les ressources communautaires et les services ambulatoires. Mais est-ce pour autant la fin de l’asile ? En retraçant l’histoire des modalités de sortie de l’asile dans le monde francophone du xixe siècle, cet ouvrage constate la nature mythique de la désinstitutionnalisation. Si on a bien fermé quelques asiles et libéré des malades mentaux pour les réintégrer dans la communauté, on était loin d’atteindre les ambitions des réformateurs de l’après-guerre. En Europe comme en Amérique du Nord, la déshospitalisation s’est en effet déployée tardivement par rapport aux proclamations théoriques et aux injonctions légales, à des rythmes différents selon les régions et de manière souvent conflictuelle. C’est sur les enjeux et les conséquences de ce phénomène historique majeur pour nos sociétés que revient cet ouvrage.
